- 2.Discount on in-patient room rates: valid in-patient room only, does not include Labor, Observation, Nursery and Economy rooms.
- 1.Bangkok Hospital Samanchan: the discount is valid on room rate, VIP and Extra rooms type and must not exceed 4 nights; the hospital reserve the right to provide only 10% on room rate which except VIP and Extra room types.
- 2.Bangkok Hospital Phitsanulok is valid on room rates which value are over THB 1,000 per night only.
- 3.Phyathai Hospital 1, 2, 3, Navamin and Paolo Hospital Group: the discount is not valid on ICU, CCU, PICU, ASU, NICU, NCU, Double, Triple and other rooms under the hospital's conditions.
- 4.The discount is not valid on meals, nursing service, hospital service, and other charges.
- 3.Discount on medical expense, doctor and dental doctor fees, special equipment, supplies, lab and other costs (Phyathai Hospital Group and Paolo Hospital Group will give the discount only for medicine.).
- 4.Out-Patient Discount on doctor fee: The discount is not valid at Bangkok Hospital Sanam Chan, Bangkok Hospital Phitsanulok, Phyathai Hospital 1,2,3, Navamin and Paolo Hospital Group.
- 5.For ambulance and Mobile ICU/CCU, the discount is applied to the transportation fees only; it does not include doctor fees, equipment or nursing fees or other service charges.
- 6.Dental discounts do not include special equipment, lab, supplies or medication costs.
- 7.Some services are not available at all hospital.
- 8.Discounts cannot be combined and may not be used with other promotions.
- 9.Only 2 membership cards / years of membership term are allowed per member.
- 10.Chivawattana may alter any privilege without prior notice.

Complementary Health Check-up Program (Please consult your physician before examination) This package is only applicable to a person with 15 years or older and must have a physically normal condition.
